Virginia clinical requirements guide
How to Get Your Immunization Records for Clinicals in Virginia
If you are a health-profession student in Virginia - nursing, allied health, medical, dental, pharmacy, PA, respiratory therapy, EMS, or another clinical program - your school or clinical site may ask for proof of immunizations before placement.
Start by looking for records from the Virginia Immunization Information System, the VDH/MyIR record portal, your current or past healthcare provider, school health office, childhood records, or military records. If a vaccine record cannot be found, an immunity titer may help document immunity for common requirements such as MMR, Varicella, and Hepatitis B surface antibody.
Practiclear is a Virginia-only service designed to support eligible titer-based screening documentation. Practiclear does not administer vaccines, provide medical clearance, or promise acceptance by any school, employer, clinical site, or compliance platform.
Common checklist items
What your program may ask for
Most Virginia health-profession programs ask for documentation of some combination of vaccines, immunity records, TB screening, and non-lab compliance items.
- MMR: vaccine record or immunity titer
- Varicella: vaccine record or Varicella IgG titer
- Hepatitis B: vaccine series and often a Hepatitis B surface antibody / anti-HBs titer
- Tdap
- seasonal flu
- COVID documentation if required by the program or clinical site
- TB screening, which is separate from immunization records
- BLS/CPR, physical forms, background checks, drug screens, or other compliance items
Requirements vary. Always follow your program's current checklist and compliance-platform instructions.
Official Virginia resources
Source 1: Virginia immunization records through VDH and VIIS
Virginia maintains the Virginia Immunization Information System, or VIIS, a statewide registry that combines immunization histories from public and private providers. VDH says the new VIIS platform launched June 24, 2025.
Virginia residents can request immunization records through VDH-managed electronic portals. VDH currently lists MyIR as a way to register and access an official immunization record, and also lists a specific COVID and MMR portal for those records. The legacy immunization record request portal has been retired.
Important limits
- State registry records may not include every vaccine you have ever received.
- Records from outside Virginia may not appear.
- Older records may be incomplete.
- Your school may require documentation in a specific format.
- A compliance platform can reject a record if the upload does not meet its requirements.
Practiclear is not affiliated with VDH, VIIS, or MyIR. We link to official sources as a student resource.
Healthcare providers and pharmacies
Primary care offices, pediatricians, urgent care clinics, pharmacies, or health systems that administered vaccines may have records, including childhood or out-of-state records.
School or college health offices
If you received vaccines through a previous school, college, or student-health center, that office may have documentation your current program can review.
Childhood or family records
A childhood vaccine card or printed record may still be useful. Check whether your school or compliance platform accepts that format.
Military records
Veterans or service members may have immunization documentation in military medical records.
Missing documentation
What to do when records are missing
If you cannot find documentation for MMR, Varicella, or Hepatitis B, an immunity titer may be an option.
A titer is a blood test that measures antibody response. Programs commonly use titers to document immunity for MMR, Varicella, and Hepatitis B surface antibody / anti-HBs.
A titer showing immunity may support documentation. A non-immune or equivocal result usually means you should follow up with your school, student health, pharmacy, health department, or personal clinician about next steps. Your program decides what it accepts.

Immunization record FAQs
Common questions about records, portals, and titers
These answers are general educational information. Always follow your program's current requirements.
Where can I get my immunization records in Virginia?
Common sources include Virginia Department of Health immunization record portals, current or past healthcare providers, school health offices, childhood records, and military records.
Can I get all vaccine records from the Virginia state portal?
Not always. Registry records can be incomplete, especially for older vaccines, out-of-state vaccines, or records submitted before modern reporting. VDH lists MyIR for official immunization records and a separate COVID/MMR portal, but students may still need provider, school, or personal records.
What if I cannot find proof of a vaccine?
For some requirements, an immunity titer can help document immune status. Common examples include MMR, Varicella, and Hepatitis B surface antibody. Your program decides whether a titer is acceptable.
Is Practiclear part of VDH, VIIS, or MyIR?
No. Practiclear is independent and is not affiliated with VDH, VIIS, or MyIR. Practiclear links to official public resources and separately supports eligible titer-based screening documentation.
Does Practiclear give vaccines?
No. Practiclear does not administer vaccines. If you need a vaccine or booster, follow up with your school, pharmacy, student health, health department, or personal clinician.
Does Practiclear provide medical clearance?
No. Practiclear provides screening documentation support only and does not provide medical clearance, occupational evaluations, diagnosis, treatment, or acceptance decisions for schools, employers, clinical sites, or compliance platforms.
How do I start with Practiclear?
Practiclear ordering starts with an eligibility check for eligible Virginia adults.
