Lab workflow

Current Labcorp path. Practiclear reviews your intake and, if appropriate, sends your Labcorp requisition and draw instructions by secure email. Confirm hours, appointment requirements, and available services directly with Labcorp before you go.

Do not go to a lab until Practiclear has reviewed your secure intake and sent your instructions. The launch workflow is manual so payment, intake, review, and lab routing can be matched before you are sent to Labcorp.

Confirm the following I am physically located in Virginia at the time of this order, or will be when the lab visit and clinician review take place. I am 18 years of age or older. (Practiclear is not currently available to minors.) I understand this is an online telehealth service reviewed by a Virginia nurse practitioner. Payment by itself does not mean a lab order or documentation will be issued. Practiclear may decline my request if it is outside scope and issue a full refund. Clinical questions outside scope will be referred to my primary care provider or a higher level of care. Full telehealth consent → I have received and reviewed Practiclear's Notice of Privacy Practices, Terms of Service, Refund Policy, and Good Faith Estimate disclosure. I understand I will be asked to identify myself with a government-issued photo ID during the medical-history intake; the order cannot proceed without identity verification.

How your information is handled at each step

On this page (eligibility check). Your service selection, fixed Labcorp lab path, and attestations are recorded by Formspree. No medical information, date of birth, or identifying detail is collected at this step.
Next step (secure payment). You enter your name, email address, and payment method. Payment and health information are handled separately — no medical history, no date of birth, no clinical detail is collected during payment. Practiclear can accept many HSA/FSA cards; individual card approval and reimbursement still depend on issuer and plan rules.
After payment (secure health form). You'll be shown or sent a link to Practiclear's secure health form. That's where you provide your date of birth, photo ID, vaccination history, and any health information the nurse practitioner needs for review.
Lab routing. Practiclear reviews your intake and, if appropriate, sends your Labcorp requisition and draw instructions by secure email. Payment, secure intake, review, and lab routing are matched manually in the launch workflow.

The final price shown before payment is also your Good Faith Estimate under the federal No Surprises Act. No payment is collected unless the secure health form handoff is available.
